ATTENTION PARENTS OF INCOMING 6TH GRADE STUDENTS 
Illinois state law requires all children entering 6th grade to present proof of a physical examination. The required physical exam form is due to Parker Junior High on June 1, 2010. Your child will not be able to begin Parker Junior High without a physical with up-to-date immunizations. If medication forms are needed, please see the Health Coordinator in your school.
