Residency Requirement
To enroll your child(ren), a variety of forms must be completed and fees collected. In addition, Flossmoor School District 161 requires you to provide the school with the following original materials to be immediately photocopied and returned to you. All items must be current and have the correct address and names.
___ Valid Illinois Driver’s License
___ Mortgage/Lease/Other
If
you own, bring in a current mortgage statement.
If
you rent, bring in your current lease showing the date of expiration, and
the name, address and phone number of your landlord. We do verify all
leases. Each year when it
expires, a new lease must be provided.
If
you live with a resident – The resident you live with must provide the
residency documents on this
page and complete an affidavit. Parent/Guardian must provide a driver’s
license, current tax return or current paycheck stub (with name and
address), and current bank or charge card statement.
If your living circumstances are different than above, you must make an appointment with the District Registrar (647-7000, 41 E. Elmwood Dr., Chicago Heights) to approve your residency documentation.

___ Original Certified Birth Certificate for Your Child (Raised seal)
___ Custody Documentation (if applicable)
Divorce
decree (front page showing petitioner, custody section(s), last page with
court stamps)
Guardianship
documents or other affidavits of custody
Current
form 906 from the Department of Children and Family Services
If you are a Foster parent, you must have placement papers from DCFS or other agency as well as meet all residency requirements.
___ Special Education Services
If your child receives special education services, please bring a current IEP for placement.
___ Health Requirements
All students must have a physical with current immunizations on file prior to attendance. This includes preschool and transfer students. A new physical examination with current immunizations is required prior to entering kindergarten and sixth grade. The school must receive the physical and record of immunizations BEFORE the first day of school. Any student not in compliance with these requirements will be unable to attend school until requirements are met and documentation is received. All children enrolling in kindergarten after January 1, 2008, and any children enrolling for the first time in a public, private, or parochial school after January 1, 2008, must have an eye examination and present proof of the eye examination to the school before October 15th of the school year.
___ School Transfer Forms
No
public school district is required to admit a new student unless they can
produce ISBE Form 33-78 from the student's previous Illinois public school
district as per Section 2-3 13a of the School Code. As per the
amendment to that code, no school district is required to admit a new
student who is transferring from an out-of-state public school unless the
parent or guardian of the student certifies in writing that the student is
not currently serving a suspension or expulsion imposed by the school from
which the student is transferring.
Please
understand that we must be assured that the school district's taxpayers are
supporting only those students who are verified residents. If you
should move during the school year, please immediately inform the office so
our records are accurate.
