Dear District 161 families and staff,

In response to a COVID-19 outbreak at Parker Jr. High School, the District is implementing a
COVID-19 community testing plan. It is of the utmost importance that we have all exposed
individuals participate in outbreak testing to ensure COVID-19 does not continue to spread
through the District and the community. We will be offering free testing to students, staff, and
family members/friends that wish to test. Parents/guardians must be present for those eighteen
years and younger.

Testing will be available at Parker Junior High School [2810 School Street, Flossmoor] on:
Monday, January 3, 2022 from 2:00 p.m. - 6:00 p.m.

We have chosen to use SHIELD Illinois as our test partner, and a patient fact sheet on the
covidSHIELD saliva test is attached. Participants should not eat, drink, use tobacco products,
chew anything, or brush their teeth one hour prior to the testing time.

If you have not already created an account with SHIELD Illinois, please register for an account at
https://shieldillinoisportal.pointnclick.com/. Please use “agency code” df5brbrj. Each adult will
need their own account. After registering yourself, please add your child as a dependent. You
may add multiple dependents in this manner. Please have this done prior to your arrival for
testing. Completing this registration provides your consent for your child to test.

If you already have a portal with SHIELD Illinois, please add this additional agency df5brbrj to all
those that will be testing.

Testing results will be sent directly through the SHIELD Illinois portal to you for yourself and any
dependents that have tested. The Illinois Department of Public Health and the local health
department will also receive results. Please notify the nurse at your child’s school if your
child does receive a positive result.
